1095-C Form Requests - 2025

Per the IRS, 1095-C forms should not be attached to tax returns beyond 2019. However, any employee who was full time or received health coverage from SRH during 2025 may request a 1095-C statement for their own records. If you would like to make such a request, please complete the request form here.
