Sponsorship Request Form
Skagit Regional Health is proud to support a variety of organizations in our community with a focus on health, prevention and wellness. If you represent a non-profit organization and are interested in applying for sponsorship, please fill out the sponsorship form and provide a letter of introduction explaining the organization’s goals and how the event or project matches with Skagit Regional Health’s mission. Skagit Regional Health will appreciate receiving the application and supplemental information a minimum of six months before the event.

A panel of staff and community members will review the applications and make recommendations to the board’s Community Outreach Committee for final approval.
