Hospice of the Northwest welcomes new Hospital Liaison

Hospice of the Northwest is pleased to introduce Eileen Gonzales, RN, who has stepped into the newly created Hospital Liaison position. In this role, Eileen partners directly with hospital providers, case managers and care teams to support timely, compassionate transitions to hospice care.

Eileen will have offices at both Skagit Valley Hospital and Cascade Valley Hospital, allowing her to meet face-to-face with patients and families to explain hospice services, answer questions, assess eligibility and coordinate admissions. After someone discharges from the hospital, there is a brief time before they are officially enrolled in hospice care. Eileen will ensure patients have everything they need during this transition.

With a diverse background in care management, hospice, home health and hospital discharge planning, Eileen will be a trusted resource for hospital staff and patients alike.
