Hospice begins with a referral
Getting started with Hospice of the Northwest is as easy as calling our Referral Center. The prospective patient, their family or physician can make the call at 360-814-5550.
Our compassionate and friendly Referral Center team is there to walk you through the process and answer questions along the way. Part of the process is making sure you qualify for hospice, which our team takes care of by reviewing medical records and confirming the diagnosis with your physician.
Hospice of the Northwest wants you to have all the information you need about hospice care and Hospice of the Northwest, so after we confirm eligibility, you will be contacted by our Referral Team to schedule an Informational Visit. This visit allows us to learn how we can help and it will allow you to ask more questions and discuss expectations.
The simple steps to getting started with Hospice of the Northwest are:
- Call the Referral Center or, if you are family member, get permission from the patient to call.
- Provide basic patient information including insurance providers and primary care physician.
- If eligibility is confirmed, accept an appointment for Informational Visit.
- After the Informational Visit, patients and/or their families can decide if Hospice of the Northwest is the right choice for them. There is no obligation or commitment through this process.
When is the right time to call the Referral Center? We recommend doing referrals as early as eligibility allows (typically, when the prognosis is 6 months) because it makes the process less stressful. Plus, when you start hospice care earlier, we’re able to increase your comfort and quality of life earlier.
