Have you had close contact within the past 14 days with a symptomatic person under investigation or with a confirmed case of COVID-19?
Appointment Cannot Be Scheduled. Please check back once your isolation protocols have been completed.
Have you received a second dose of the Moderna or Pfizer vaccine?
Appointment Cannot Be Scheduled. This page is for third dose scheduling.
Are you experiencing any of the following symptoms: fever, cough, shortness of breath, runny nose, sore throat, severe headaches, loss of taste or smell, chills in the absence of fever, vomiting, diarrhea, muscle pain, or new fatigue?
Appointment Cannot Be Scheduled. Please discuss your symptoms with your primary care doctor to determine when it would be appropriate to get the vaccine.
Has it been at least 28 days since your second dose?
NOTE: You must schedule your third dose with the same vaccine type (Pfizer or Moderna) as you received for your first and second dose.
If you are under 18 years of age, a parent or guardian must accompany you to your appointment.
