Vaccine appointments are available five days a week at Skagit Regional Health - Smokey Point in the Family Medicine department for those 6 months of age and older.

First Dose (Moderna 6 months - 5 years)

Schedule your first dose of the COVID-19 vaccine. If you are under the age of 18, a parent or guardian must accompany you to your appointment.

First Dose (5+)

Schedule your first dose of the COVID-19 vaccine. If you are under the age of 18, a parent or guardian must accompany you to your appointment.

Second Dose (6 months - 5 years)

Schedule your second dose of the COVID-19 vaccine for those 6 month - 5 years of age. If you are under the age of 18, a parent or guardian must accompany you to your appointment.

Second Dose (5 - 11)

Schedule your second dose of the COVID-19 vaccine for those 5 - 11 years of age. If you are under the age of 18, a parent or guardian must accompany you to your appointment.

Second Dose (12+)

Schedule your second dose for those 12+ years of age. If you are under the age of 18, a parent or guardian must accompany you to your appointment.

Third Dose (Immunocompromised 5 - 11)

Schedule your third dose if you are immunocompromised and are 12 years of age or older. There must be a minimum of 28 days between your second and third dose.

Third Dose (Immunocompromised 12+)

Schedule your third dose if you are immunocompromised and are 12 years of age or older. There must be a minimum of 28 days between your second and third dose.

Booster Dose (5 - 11)

Schedule your booster COVID-19 vaccines. Booster doses are only available for patients who have completed their primary COVID-19 vaccine. First boosters are available for those 5 years of age and older. Second boosters are available to those 50+ or immunocompromised. Those 12 - 17 and immunocompromised may receive a second Pfizer booster. Pfizer is the only COVID-19 vaccine approved for those under 18.

Booster Dose (12+) - Scheduling for this vaccine dose is temporarily closed.
